Welcome to the Middleton-Cross Plains Area School District! We are excited to welcome your kindergarten student as they become part of our community of learners. 

All of our MCPASD schools have a health office that is staffed by a School Nurse (RN) and a Health Assistant (Certified/Registered Medical Assistant or Nursing Assistant). The school health team is there to assist your student with illness, injuries, taking medication, or providing emergency care during the school day. Someone is present in the school health office at all times.

For new kindergarten students in MCPASD, complete the following two forms and turn them in at your school health office. You will be asked to provide information about any health conditions and vision, hearing, and dental screening information.
