The registrar's office is typically open Monday through Friday from 7:30 a.m. to 4 p.m. during the school year. The District Services Center, which is where the registrar's office is located, has been closed since mid-March due to COVID-19. DSC is expected to reopen on Monday, July 6. Hours are more limited in the summer. Their office is usually closed on Fridays in July and open from 7:30 a.m. to noon on Fridays in late June and early August. K-12 registration:

Who is Eligible to Attend:
In accordance with the Middleton-Cross Plains Area School District's Admission Policy, any child who physically resides in the District for a purpose other than school attendance may be considered a resident student and may be eligible for tuition-free admission to the District's schools.

Students will be assigned to the attendance school, based on address and the attendance boundaries defined by the district. However, the district reserves the right to assign students outside of the attendance boundary based on enrollment, class size or program needs if necessary.

Students must wait up to 48 hours after their registration forms are submitted before they can attend school.

What is needed to register:
A parent or guardian is required to provide to the Registrar's Office the following information:

1. Proof of Residency (one of the following):

  • Lease Agreement: name, address, effective date and all parties signatures

  • Accepted Purchase Agreement: name, address, occupancy or closing date and all signatures; additional proof may be requested once you have occupied the residence

  • Building Contract and Building Permit: name, address, projected completion date and all signatures on contract; additional proof may be requested once you have occupied the residence

  • Current Utility Bill: natural gas; electric or city/village water. Name, address and current service address. We do not accept insurance, telephone, cable bills or closing statements as proof of residency

2. Proof of student's age (one of the following):

4. Student Enrollment Form 
You can fill out the 2020-21 Fillable Enrollment Packet, which includes the Student Enrollment Form, registration procedure, transfer of records request (if applicable), resident status verification form (if applicable), change of address (if applicable), withdrawal notification (if applicable), free or reduced meal waiver (if applicable) and McKinney-Vento Act eligibility form (if applicable). A 2020-21 Enrollment Packet in Spanish is also available.

4K Registration

In addition to the proof of residency and proof of age (see #1 and #2 above) you will need to download the 2021-22 4K Fillable Enrollment Packet or the 2020-21 4K Fillable Enrollment Packet. A Spanish verison for 2021-22 and a  Spanish version for 2020-21 are also available. The packets will also available on the 4K website.

Information for Incoming Kindergarten Students new to the District

Free & Reduced Meals Information
