Note: This year is a Sunday Ceremony. Historically Middleton's Graduation was on Sundays, post COVID we were moved to Saturday at Alliant and this year we were able to lock in a Sunday.

• If you are 18 years old and would like your name changed for you Diploma please fill out this form. Know that if you fill out the form AFTER May 1st you most likely will not have your name change on the diploma that is handed to you at Graduation but it will come in the mail to your home.

• If you would like your Name Changed to be read at Graduation and you are 18, please email lbakke@mcpasd.k12.wi.us with your legal name and your preferred name so it can be incorporated into practice and the ceremony.

• If you are under 18 and want any of these items to happen, you will need Parent Permission with the Name Change Form (if you want Diploma also changed) or Parent email lbakke@mcpasd.k12.wi.us with the Name to be Read at Graduation.
