WINTER SPORT MEETINGS FOR ATHLETES:
Winter sport meetings will take place on October 16, 2019 immediately following school. Fall sports will delay their practices by 30 minutes. These meetings are for the athlete to sign-up and acquire information about the sport that they wish to play. The designated locations are as follows:
- Boys Basketball - Varsity Gym East
- Girls Basketball - Varsity Gym West
- Boys Hockey - Small Gym
- Coed Ski - Field House North
- Coed Snowboard - Field House South
- Wrestling - Wrestling Room
- Boys Swim and Dive - Pool Lobby
NOTE: Girls Hockey will be meeting on October 21, 2019 at 7:30 p.m. @ MIA and Gymnastics has already met and if you were not in attendance, please email the head coach. Her information can be found on the Gymnastics page.
FeePay for 2019 Winter Athletes
FeePay for the 201920 Winter Sports Season is officially open. Please be sure that your athlete is signed up via FeePay prior to the first day of practice/tryouts. Physicals must be on or after 4-1-18 to be valid. You can check with the athletic office on the date of your last exam.
In accordance with the MHS philosophy of athletics and our desire to have as many students as possible participate in the athletic program at MHS, we encourage coaches to retain as many athletes as possible (most sports utilizing a rubric system) without affecting the integrity of their sport. Time, facilities, equipment, personal preference, and other factors place limitations on the most effective squad size for any particular sport. When developing policy with respect to squad selection, we strive to maximize opportunities for our athletes without diluting the quality of the program. Most of our athletic teams do not cut at the Freshmen level, however in Volleyball, Basketball, Soccer, and Hockey the above mentioned factors may force us to cut from year to year in those sports. Remember, if you get cut from one of these sports it may open the door to a new opportunity.
2019-2020 School Year Free Physicals for Uninsured or BadgerCare High School Athletes:
Attention: All Uninsured High School Athletes
On Tuesday, Aug. 13, 2019 from 6 - 9 p.m. Access Community Health Center, 2202 S Park St., Madison, WI will be offering free sport physicals to uninsured athletes.
Last names A-I (arrive @ 6 p.m.)
Last names J - R (arrive at 7 p.m.)
Last names S - Z (arrive no later than 8 p.m.)

In order to receive immunizations, a parent MUST be present. Please bring immunization records with you.
Female athletes please wear tank tops.

NOTE: Athletes that begin tryouts and practices prior to August 12th, you will need to acquire your physical elsewhere. Concentra on Junction Road in Madison does offer physicals for a nominal fee on a walk-in basis.

INCOMING FRESHMAN AND NEW ATHLETE ALL-SPORT MEETING:
There will be a parent and athlete meeting for all incoming freshman and/or new MHS student-athlete interested in playing ANY sport, during any season, for the 2019-20 school year.
The meeting is scheduled for May 30, 2019 from 6 - 7 p.m. in the Performing Arts Center at Middleton High School. Immediately following that meeting there will be break-out sessions for the fall athletes only. The designated locations of those break-out sessions will be provided at 7 p.m. The winter/spring athletes and their parents may depart at 7 p.m.
Attendance at this meeting is highly encouraged for both parent and athlete. There will be significant information provided, as well as an overview of the student athletic code.
FOR THOSE FRESHMAN AND PARENTS THAT WERE UNABLE TO MAKE THE MAY 30 MEETING, SEE TOP OF PAGE FOR THE SPORT ORIENTATION DOCUMENT.

2019 FALL SPORT SIGN-UP:
THE FALL SPORT MEETINGS FOR RETURNING MHS ATHLETES WILL TAKE PLACE ON THURSDAY, MAY 30 IMMEDIATELY FOLLOWING SCHOOL. THIS IS FOR THE ATHLETE ONLY. ROOM LOCATIONS CAN BE FOUND ON THE ANNOUNCEMENTS AND ON THE TV'S WITHIN THE SCHOOL. PARENTS ARE NOT TO ATTEND.
All athletic forms, except the medical clearance form, will be submitted electronically when you sign-up to register your student(s) via FeePay. The forms will auto-populate at the time that you choose the fall sport or any year long club that charges a club user fee.
For those that do not have access to a computer, the athletic office will accept hardcopy forms. We anticipate 90% of all required forms, EXCEPT THE MEDICAL CLEARANCE FORM, to be submitted electronically.
NOTE: ALL MEDICAL CLEARANCE FORMS MUST BE TURNED INTO THE ATHLETIC OFFICE PRIOR TO THE FIRST DAY OF TRYOUTS/PRACTICE OR YOUR STUDENT WILL BE INELIGIBLE TO PARTICIPATE UNTIL A VALID DATE IS ON FILE.
You will be unable to register for a winter or spring sport via FeePay until a pre-determined date established by the district. A district correspondence will go out in October and March informing you that you are able to sign up via FeePay. Again, at the time you choose the sport you wish to sign your student up for and the forms needed will then auto-populate. These forms include the consent form, concussion form, emergency contact card and transportation/driving waivers. Again, the only form that you will need to turn into the athletic office will be the MEDICAL CLEARANCE FORM which you can now find here on the MHS website under Athletics<Athletic Information<Forms.
