Follow the directions to set up a new family account and pay for fees and add money to your student’s lunch account. Step 3: Fill out questionnaire for each student

Please click on the link below and fill out a questionnaire specific to each student. You will need to enter their student ID number to gain access.

Qualtrics survey.

As  part of the annual student verification efforts, the District is asking families to take a few minutes to complete this Qualtrics survey. You will be asked to complete up to eight sets of questions. If you have several students, you will need to answer the questionnaire for each particular student. This important data includes privacy limitations, student insurance, medical data, and other pertinent information that is needed prior to the start of each school year
The last section includes a question about your family’s plans regarding virtual learning. While the survey will remain open through August, District administrators are requesting that families complete it by Aug. 10 in order that cohorts in schools can be developed along with making final determinations on transportation once MCPASD returns to in-person instruction for students in school buildings.
If you are new to our District, you have likely already provided much of the annual student verification information. However, please complete the Fully Virtual Option section of the survey.
If you have questions regarding registration or address changes, please contact the registrar’s office at 829-9031 or send an email to reg@mcpasd.k12.wi.us.
If you have questions regarding school-specific topics, please reach out to the office staff at your child’s school. Contact information may be found on the District website.
If you have technical issues with the survey, please email portal@mcpasd.k12.wi.us or call 829-9600

Helpful Hint: Before you begin, gather your information, such as name of Doctor/clinic phone number, emergency contact information, list of medications, etc

Bus routes will be determined at a later date.  The transportation department will be in touch with families when the routes are completed.
