Our District participates in the Wisconsin Public Employers Group Life Insurance Program, which is administered by the Department of Employee Trust Funds and provides group term life insurance coverage for state and local employees. The coverage is underwritten by Minnesota Life Insurance.

Information about Electronic Signatures
For most forms, ETF will accept an employee’s and employer’s electronic signature unless otherwise noted.
An electronic signature will be recognized as a valid signature as long as the following criteria is met:
The applicant is eligible to add, modify, or cancel coverage.
The application is complete.
The application is received by the applicable eligibility deadline.
Life Insurance only: The application is accompanied by an email from the employer stating that they have confirmed the identity of the individual submitting the application, or the form is submitted through an authenticated electronic system.
ETF will continue to evaluate electronic forms and convert them to fillable PDFs as appropriate.
