It is important to notify the Benefits Office if you believe you will qualify for Long Term Disability Income benefits due to an extended illness or condition.
A Disability claim packet should be initiated within 30 days of the date the Elimination Period ends, if that is possible. The claim packet is in 3 parts.
- Employer (MCPASD Benefits Specialist completes)
- Employee, and
- Attending Physician
Disability Claim Form Packet - NOTE: You will receive this form packet from the Employee Services Department when they have been informed an employee intends to file for disability benefits. The entire claims packet should be submitted to the carrier.
